Example Citanest 002 scratched out to Lidocaine 002 (these are just examples) Names are changed and the dosages.
Example: What if you had a tooth extracted and the dentist forced you to sign it, and you refused to and it is NOT signed and she pulled out your tooth?
Also, if you were hospitalized after dentist gave you anethesia at her office, and you got the dental records, and it says "you were NOT even there that day no record of being there" is this a SERIOUS offense on the dentist's part?
